The hero went through a long journey, harsh training and gained many power-ups, Mid Season Upgrades, and Super Modes. In the climactic battle they use everything they gained and... lose!? However they then somehow manage to defeat their opponent in their base form instead.
It is rather common for the hero to defeat their enemy in their base form, or in some extreme cases without using their powers at all, instead of fighting them with their supposedly superior and stronger forms or powers. Reasons can be that the heroes are just too tired to transform into their stronger forms, which in some cases is the reason for their exhaustion, the villain can only be damaged by a specific weapon or attack which only the "base form" has, the hero wants to assert their dominance over their opponent, or the event serves as a Book End. This can also be used as An Aesop to the hero to teach them that they can't just rely on their new power alone and also need to use their skills.
Another question that comes is why the villain is unable to defeat them despite the fact they were able to completely demolish them in their Super Modes. Reasons can be that the fight devolves into a Combat Breakdown, the villain is weakened, is just as exhausted as the hero, Heroic Spirit is in effect, they forgot to grind themselves, their weakpoint is being exploited, the base form gets a new weapon, attack, or actually empowered inside, or just Rule of Cool is in effect.
Also note that this doesn't only apply transformations, but various kinds of power-ups which can include techniques, weapons and Mecha Expansion Packs.
Compare and contrast to So Last Season, which this trope plays straight and subverts at the same time. Related to Combat Breakdown, Brought Down to Badass, Power Nullifier, Wrecked Weapon, Heroic RRoD (or Fuel Meter of Power), Cherry Tapping and You're Nothing Without Your Phlebotinum which all can be reasons why this comes into play.
Compare with Bishōnen Line and No-Gear Level.
Note: Due to its tendency to be used as a way to Bookend the series, nearly every entry here is a spoiler by default.

After Ultraman King summons copies of every form Ultraman Geed used through the series to reduce Belial Atrocious back to his base form. The clones fade away with the final battle becoming an emotional showdown between Geed Primitive, his initial form, and Belial, which ends in a Beam-O-War that Geed wins. As part of the symbolism, Primitive is also the form that resembles Belial the most with said victory showcasing Geed's status as his father's Superior Successor and Redeeming Replacement.

Kamen Rider Revice: Revi and Vice's Super Mode had always been portrayed as extra armor on top of the base form, so the final fight between them just had both take hits that shattered the outer layer and left the original forms intact. It helps that they had already resolved their differences at that point and the "fight" had shifted to playful roughhousing, so it didn't matter any more that they weren't using their full power.

Yu-Gi-Oh! ZEXAL: Throughout the series, Yuma's Signature Mon Number 39: Utopia has received many upgrades yet the original managed to prevail and finish off the fights in which its upgrades were defeated:
In the last battle against Number 96, Utopia Ray Victory fell to 96's three Chaos Numbers, but after defeating two of them with Utopia Ray equipped with two new ZEXAL Weapons, the original Utopia is used to finish off Number 96 by destroying its Chaos Number form.
After having his Utopia Ray Victory rendered powerless by Don Thousand's Numeronius, later Yuma ends up finishing off Don Thousand and his even more powerful monster with 100000 ATK/DEF with Number 39: Utopia, whose ATK is boosted to 204000 by various effects.
In the duel against Nash, Yuma was forced to banish Utopia Beyond and special summon Utopia in its place to save himself. Utopia then managed to end the emotional duel through preventing its own attack causing Nash to lose to the effect of Glory of the Seven Emperors, which would have inflicted all the damage he would take to Yuma and changed his Life Points to said amount.

Bleach: In the final arc, Ichigo discovered the true nature of his zanpakuto, culminating in a new, dual-wield Zangetsu as well as a "Merged Hollow" form he can completely control. However, Yhwach uses his Reality Warper abilities to shatter Tensa Zangetsu and absorb Ichigo's Quincy and Hollow powers. It's only with the timely intervention by Tsukishima that his Bankai is repaired, but he is still unable to use the full extent of his power. When Ichigo goes for the critical blow against Yhwach, the Quincy attempts to break Zangetsu again, but it's revealed that the original Zangetsu (his Shikai, no less) was hidden within the new one, and Ichigo uses it to bisect Yhwach and put him down for good.

Kamen Rider Wizard: Haruto initially battles Gremlin who has been enhanced with the Philosopher's Stone with his Infinity Style in the final fight, but is overpowered by the Phantom and forced down to Flame Style. Despite having every reason to give in to despair, he makes one final guard (which destroys the ring on that hand) then throws his first actual punch all season long into Gremlin and manages to tear out the Philosopher's Stone, converting it into the Hope Wizard Ring and gaining enough magic power to destroy Gremlin once and for all.

Tengen Toppa Gurren Lagann: You can expect this to happen a lot thanks to Spiral Power.
Lordgenome's Lazengann was able to No-Sell Gurren Lagann's Giga Drill Breaker and cripple the Gurren part forcing Lagann to continue the fight, but its Finishing Move was overpowered by Lagann Impact.
Throughout their war against the Anti-Spiral, Simon was able to gain Gunmans whose heads are large enough to fit the previous Gunman till he gains the titular mecha, but in the final battle it is not the one that finishes them off. However it goes differently by adaptations:
In the original anime, the mechs kept spitting each other out to get Gurren Lagann close enough for Viral to throw Lagann towards the Anti-Spiral who in turn defeats them with a Lagann Impact.
In the movie retelling the mechs kept spitting each other out, but the sequence is extended — Viral detaches Lagann to get it closer to the Anti-Spiral, and Lagann itself tosses Simon out to beat up the Anti-Spiral in a fistfight.

Naruto:
During Naruto's battle with Pain, Naruto begins the battle using his newly-mastered Sage Mode until he expends all of his energy as well as the clones required to make more. Pain manages to evade both of Naruto's Rasen-shuriken attacks, and knows that Naruto won't have the time to gather enough energy to reenter Sage Mode. Just when he thinks he's won, though, Naruto reveals that he used the original trick he's known for: making a LOT of copies of himself to attack Pain all at once. While fending off the clones, he notices Naruto making a basic Rasengan but thinks he's safe since that jutsu can't be thrown—until Naruto simply uses his clones to throw himself at Pain and deliver the finishing blow.
Naruto's and Sasuke's final battle starts with them using their most powerful techniques and transformations, but in the end they also became exhausted till a point they could barely stand straight and finished their fight in their normal forms.

Dragon Ball:
Despite the power Goku gains by transforming into a Super Saiyan 3 in Dragon Ball Z, it fails to defeat Kid Buu due to how much it drains his stamina. Instead, Goku defeats Buu by throwing a Spirit Bomb at him, then using his Super Saiyan form to allow him to push the bomb into Buu with more force, allowing it to overcome Buu.
In Dragon Ball GT Goku gained the Super Saiyan 4 form which was able to defeat Baby, but it is unable to defeat the Arc Villains of the later two arcs, Super 17 and Omega Shenron, and instead he finishes them off in his base form. In the former's case he exploited Super 17's weakpoint and in the latter he managed to use the Spirit Bomb to finish him off.
In Dragon Ball Z: Battle of Gods and its corresponding saga in Dragon Ball Super, Goku starts his fight with the God of Destruction Beerus in his new Super Saiyan God form. As the battle rages on, the form expires and Goku reverts back to his base form. When Beerus tries to make good on his promise to destroy Earth in the event of Goku's defeat, Goku manages to briefly reattain his Super Saiyan God form long enough to absorb Beerus's attack.
In the climax of the Tournament of Power of Dragon Ball Super, Goku gained Ultra Instinct, but before he's able to finish off Jiren, his body was unable to handle the new power he got. In the end he fights against Jiren along with Frieza who also can't transform into his Golden Form and 17. However they were still able to defeat the equally exhausted Jiren. In fact, throughout the final seconds of the fight, he's seen flickering in and out of regular Super Saiyan state, showing he's so out of juice, he can't even maintain THAT.

Mobile Suit Gundam 00 did this in both season finales.
In the first season finale, Setsuna first engaged the Alvatore using the GN Arms Type-E but combat damage forced him to ditch it after wrecking the Alvatore and fight the Alvaaron with only the Exia's weapons. After expending Trans-Am and the Exia's entire melee weapon loadout on turning the Alvaaron into a mobile suit pincushion, Setsuna wins... only for Graham Aker to suddenly show up, not in a Gundam Throne or even a GN-X, but a Flag customized with Gundam equipment, and proceed to fight the exhausted and nearly-unarmed Exia to a Mutual Kill.
In the second season finale, Setsuna and Ribbons go toe-to-toe, mutually destroying each other's weapons one by one until Ribbons manages to cut off one of the 00 Raiser's GN Drives, though not without taking crippling damage to the Reborns Gundam in the process. Fortunately for Ribbons, he happens to stumble upon the very same 0 Gundam he piloted all the way back in the first season prologue flashback which Setsuna's allies used during the battle but had to abandon when its Phlebotinum Battery ran out, a problem Ribbons swiftly fixes with the GN Drive he took from the 00. When he goes back to finish off the crippled 00, however, he discovers that Setsuna bailed out and plugged the other GN Drive into none other than the very same Gundam Exia he used in the first season, with which Setsuna proceeds to tear the even more obsolete 0 Gundam a new exhaust port.

The original series had Atem resorting to sacrificing Obelisk (along with Gazelle the King of Mythical Beasts) to summon Dark Magician with Magical Dimension in his duel against Yami Marik, after failing to defeat Ra with both Slifer/Osiris and Obelisk due to Ra's status as a Top God. With Dark Magician and Dark Magician Girl present on the field, Atem then proceeds to destroy Ra with Magical Dimension's additional effect in manga which treats the monster target as being destroyed by battle, while in the anime, he uses Ragnarok to banish every monster he has outside the field to destroy Ra.

Asura's Wrath: Asura has many Super Modes, which grants him transformations such as giving him extra arms or making him go One-Winged Angel. In several fights, though, Asura got damaged to the point of losing his transformations, or even rendered limbless. He still manages to finish off his enemies anyway, as losing those transformations don't reduce his accumulated Wrath Mantra.
Wyzen eventually turns into planet-sized Gongen Wyzen who tries to squash Asura in his six-armed Vajra form with his Finger Poke of Doom. Asura fights back by delivering Rapid-Fire Fisticuffs to said finger, and ends up breaking all but one of his arms, but he manages to kill Wyzen using his last remaining arm.
Augus at first No Sells Asura's punch while he's in his Vajra form, and Asura proceeds to use six-armed Vajra form to fight him. Augus eventually extends his sword to the point of piercing Gaea itself, breaking all of Asura's arms in the process except one while blocking the blade. Asura uses his last remaining arm to break the sword (along with said arm), and grab it in his mouth to cut Augus in half.
After getting Karma Fortress' Mantra Reactor to make his body able to handle his enormous wrath, in the Final Battle, right after Chakravartin entered their own Super Mode, their Finger Poke of Doom instantly reduced Asura from his strongest form to his base by breaking all of his oversized arms. By relying on his agility and fueled by fatherly Unstopppable Rage, Asura was able to kill Chakravartin.

Kamen Rider Ex-Aid manages to do this twice.
First, when Cronus goes One-Winged Angel by merging with Gamedeus, the Riders turn back to their long-unused Super-Deformed Level 1 forms. While bulky and not particularly strong, Level 1 was specially designed to separate humans from Bugsters.
After taking Gamedeus out of the picture, Cronus is defeated by Ex-Aid, Brave, Snipe, Lazer in their Level 2 forms and Genm in his Level 0 form. In this case it's because Cronus has been weakened by Parad's sacrifice and is unable to use his pause ability. Other than how Cronus had already been established to be Unskilled, but Strong, Parado's Heroic Sacrifice also suppresses the virus on Cronus, making him gets tossed around between the five Riders and ultimately goes down pretty easily.

Metal Gear: Final battles in the series (after the 8-bit era) tend to be this way. Regardless of the amounts of abilities, weapons or Humongous Mecha the player has used or acquired throughout each game, the final boss fight always comes down to a simple one-on-one duel. Metroid II: Return of Samus: The easiest way for Samus defeat the Metroid Queen is to shoot the Queen's mouth when she snaps it at Samus, leaving stunned and wide open so Samus can Morph Ball roll into the Queen's belly and bomb the Queen from the inside. Otherwise it takes a lot of missiles to the Queen's head to defeat her, and with the missiles slow flight speed and the head constantly moving, biting at Samus, and occasionally spitting homing fireballs, the fight becomes an exercise in frustration.

Digimon Data Squad: In addition to Digimon evolving through Rookie, Champion, Ultimate and Mega forms as they grow stronger, this season adds an additional "Burst Mode" which causes fully-evolved Digimon to change colour and become even more powerful. In the final battle, Yggdrasil defeats all the heroes and forces ShineGreymon Burst Mode to revert all the way back to its Rookie form Agumon. It is then defeated when Agumon manages to summon a Battle Aura similar to Burst Mode without transforming.

Hero: 108: When Spotter invades Rabbit Castle with his army of mantisses. Jumpy Ghostface trains himself with Beetle King and obtains a shell, which gives him flight, to use against his former competer for the crown. During their battle, Jumpy's beetleshell breaks when Taking the Bullet for Mystique Sonia, but in the end he is able to subdue Spotter with just his jumping rope as to show that he truly is much more worthy as Rabbit King even without his new skills.

Mass Effect: In-Universe, most soldiers or armed-and-dangerous persons are covered with kinetic barriers: personal force-fields that protect them from any incoming weapons fire that moves above a certain speed. With sufficiently strong barriers, this makes someone pretty much immune to any form of damage...except getting punched, kicked, clubbed or stabbed. Since the barriers only activate against high-velocity weapons like lasers, bullets and missiles, ordinary melee weapons aren't affected at all. This is exploited in Mass Effect 3 with the creation of Omni-blades, which allows a Hard Light machine normally used to create tools to instead create close-quarter blades that can cut through any type of armor and bypass shields.

Kamen Rider:
Kamen Rider Kuuga: Yusuke first fights N-Daguva-Zaeba in Kuuga Ultimate Form, but they both de-transform after they break each other's Transformation Trinkets which ends with Yusuke killing him.
Kamen Rider Ryuki: In all of his fights against Odin, Ren first used his Survive Form against him, but is forced to revert to his base form after being damaged and overwhelmed by Odin's Game Breaking abilities. However he is able to kill Odin the first time by catching him off-guard by impaling him and the second time by hitting him with his Final Vent when he's distracted by Yui's scream.
Kamen Rider Den-O: After the Death Imagin managed to defeat both Den-O and Zeronos in their Liner and Zero form respectively. He ends up being defeated by Sword Form's finisher which is empowered by all of his allies, but not before also being attacked by Rod, Axe, Gun and Zeronos Altair form.
Kamen Rider Fourze: In the end Gentaro defeats Sagittarius with a souped up finisher in Fourze's Base Form after bombarding him with every form he had, including pummeling him as Gentaro, not Fourze.
Kamen Rider Gaim: Played With, as the form is clearly stronger, but the Super Mode for the titular character forgoes the use of Genesis Core (a component of the superior Genesis Driver) which he uses for Jimber Arms forms, only making use of the Sengoku Driver (while the final form materializes Senyo Joint for use).
Kamen Rider Ghost: After defeating the Great Eyezer in its giant form, it reverts back to its smaller form sans its parka. Takeru, having lost access to Mugen Damashi after besting the Eyezer Giant, opts to fight it with his basic Ore Damashi and manages to destroy it with the help of the Parka Ghosts.
Kamen Rider Build: After both undergoing Combat Breakdown, Sento attacks Evolt with RabbitDragon's Finishing Move which is esssentially souped up version of RabbiTank's that even changes back to that form due to the amount of power it uses.
Both Kamen Rider Zero-One and Kamen Rider Saber use a different spin on this, giving their heroes an 11th-Hour Superpower in the final episode; a new Transformation Trinket that grants a form with the power of the Super Mode but the appearance of the base form.
Kamen Rider Geats: After Big Bad Suel effortlessly defeats Tycoon, Na-Go and Buffa in their strongest forms and De Powers them to boot, Geats arrives having come Back from the Dead, restores their Rider powers with their base Buckles, and proceed to curb-stomp Suel with said base forms before Ace finishes off the fight with his Super Mode.

Yu-Gi-Oh!: In general despite the amount of new cards or upgrades they gain, you can expect the main character to finish the duel with their regular ace instead.
Yu-Gi-Oh! GX:
In both of Jaden's duels against Sartorius, a fusion of his Elemental HERO Neos is destroyednote Grand Neos in the first & Magma Neos in the second, he brought back the original with Reverse of Neos, who then finished Sartorious off.

Yu-Gi-Oh! 5Ds:
In his duel against Z-ONE Yusei was able to summon Shooting Quasar Dragon which destroys all of Z-ONE's monsters and itself in a mutual kill. In the end, Yusei defeats Z-ONE and his even more powerful monster with Stardust Dragon empowered by Converging Wishes (which basically empowers it with all of Yusei's dragons).
In their last duel, Yusei's Shooting Star Dragon and Jack's Scar Red Nova Dragon both destroy each other in a Mutual Kill, but Jack was able to summon Red Daemon's Dragon, while Yusei had various low-level monsters on his field and then defeated Jack's ace monster with Junk Warrior, the first Synchro Monster used in the series and other ace.
The manga's last duel had Yusei bringing back Stardust Spark Dragon after Rex destroyed his Stardust Sifr with his final monster Ultimitl Bishbaalkin via Sifr's final effect, which ends up depleting Rex's Life Points with the help of Gathering Light and Final Cross.
Yu-Gi-Oh! ARC-V:
In Yuya's second and third duels against Jack, he gained many new powerful monstersnote In the second duel he got Enlightment Paladin and in the third duel he gained the other three Dimension Dragons and yet Jack was finished off by Odd-Eyes Pendulum Dragon. Yu-Gi-Oh! VRAINS: In his last duel against Ai, Yusaku uses Firewall Dragon Darkfluid, the monster that defeated Bohman, and Accesscode Talker and while the latter was able to tie against Ai's The Arrival Cyberse @Ignister. Yusaku defeats him with Decode Talker the first Link Monster he used in the series, while Ai tries to defeat him with its Evil Counterpart, Dark Templar @Ignister.

Ultra Series:
Instead of using his combat-oriented Corona or the more powerful Eclipse form, Ultraman Cosmos uses what's essentially his Luna Form empowered by the desire of his host Musashi to purify Chaos Header instead of destroying him.
Ultraman Z had Z, badly wounded, dazed, and knocked out of Delta Rise Claw into his default form after the sacrifice of the Beliarok in a second attempt to prevent Destrudos from using the D4 Ray. He eventually gets it together and fights Destrudos evenly, then clashes in a Beam-O-War with the D4 Ray and his Zestium Beam, and overpowers it to put an end to Celebro's Civilization Self-Destruction Game.
Ultraman Decker: The final battle against Mother Spheresaurus sees Decker start in Dynamic Type, but at the climax he reverts to Flash Type for one last Beam-O-War, briefly gaining a Golden Super Mode as he overpowers Mother Spheresaurus.

Super Sentai:
In Hyakujuu Sentai Gaoranger and its American adaptation, Power Rangers Wild Force, Ultimate Org Senki/Master Org managed to destroy all the Power Animals/Wild Zords except for the original five who in turn merge together without the Rangers and seemingly managed to succeed at defeating him... only for the villain to reform and kill them as well. However, they still did better than the other Power Animals/Wild Zords including their god!
Samurai Sentai Shinkenger: Dokoku was able to gradually reduce Samurai Ha-Oh to Shinken-Oh during a Unflinching Walk who in turn was able to kill him in one attack which the Shinkengers poured all their strength into the blow.
Tensou Sentai Goseiger: Brajira was able to overwhelm the Goseigers in their Super Forms, but was then subsequently defeated by their normal forms after they regain their fighting spirit and creative use of their remaining weapons.
Kishiryu Sentai Ryusoulger: The team starts their final battle against Eras with Ryusoul Red using the Max Soul and the others using the armors of the KyoRyuSouls while piloting the Kishiryu. However in the end the weakened Eras is defeated by Ryusoulgers (except for Koh who was temporarily killed) in their base form due to their Ryusouls only having barely enough power to transform after the sacrifice of the Kishiryu.

Scryed: Kazuma and Ryuho finally have the battle they've wanted since the beginning of the series and start with their original Alter forms. At the fight's apex, they've started using the strongest forms of their Alters and are literally fighting in outer space. That clash leads both of them to crash back to Earth with their power mostly depleted—whereupon they begin fighting with their bare hands and no advanced abilities. When both are pushed to the absolute limit, they summon the smallest, most basic portion of their Alter that they can still manage and deliver one final blow....which still ends in a draw.

Inazuma Eleven: The goal of the 2nd game was becoming stronger and learning new techniques and in the game against Dark Emperors, Tachimukai's Mugen the Hand is unable to stop Dark Phoenix, but somehow Endou's God Hand was able to stop it despite supposedly being much weaker.

Pokémon: The Series has Ash and Leon's final battle where Ash is allowed to use all three of Mega Evolution, Z-Moves, and Dynamax, with the latter being able to be used twice thanks to Eternatus, the double use of Dynamax also being granted to Leon. However the final battle ends up being between Ash's Pikachu and Leon's Charizard in their base forms, Pikachu having used both 1,000,000 Volt Thunderbolt and Gigantamax while Leon's Charizard had previously Gigantamaxed as well.

Yu-Gi-Oh!

Metroid:
Metroid: After acquiring weapons like the Wave Beam and Screw Attack, the only way for Samus to defeat Mother Brain or the Zebetites that guard her pod is with missiles. Lots, and LOTS of missiles.
Super Metroid: Mother Brain is immune to any weapons except missiles or the Charge Beam (although this time, most of Samus's upgrades stack). Subverted in the final phase of the boss fight, where the Giant Metroid takes Mother Brain's Hyper Beam and gives it to Samus, which completely wrecks Mother Brain's shit.
Metroid: Other M: In the epilogue, after defeating Phantoon and retrieving Adam's helmet, Samus has to hurry back to her ship and fly away before the Bottle Ship explodes. Yet for some reason she chooses to do so while in her Zero Suit and only wielding a stun pistol, which is problematic since Zebesians are all over the place shooting at her while she's running towards her ship.

Gaiking: Legend of Daiku Maryu: Gaiking is composed of three units that dock together - a cockpit, arms and legs - and receives a Mid-Season Upgrade in the form of Gaiking the Great, which combines Gaiking's cockpit with the stronger arms and legs of its sister units Raiking and Vulking. In the final battle, Daiya achieves victory by having Gaiking the Great hold the opponent down, then ejecting its cockpit from the formation and reassembling the original Gaiking to unleash all its power at point-blank range.

Kirby: Planet Robobot: Before the battle begins against Star Dream, Kirby's Robobot armor absorbs the Halberd to take the fight into space. After merging with the Access Ark and still losing, Star Dream fires one last beam against the Robobot's Halberd mode as a last ditch effort, forcing Meta Knight to eject Kirby with the Robobot Armor for what is essentially a Giga Drill Break in order to defeat the rogue machine for good. The fight with Star Dream Soul OS takes it even further by having it inhale Kirby after he defeats its third form, forcing Kirby to take the battle on foot and destroy the heart of the machine with whatever Copy Abilities the player has on hand.
