Sometimes, the most powerful fighter will not be equipped with such Stock Superpowers as super-strength, or Nigh-Invulnerability. Instead, they have the power to see into the future.
Why is this useful? Because it lets them know your next move, and plan accordingly. Being able to see even a few seconds into the future can be a huge advantage in a fight and can let someone get the edge over opponents that might have more skill or experience and just the shock of having their moves foreseen can cause them to slip up and make mistakes. However, although it can give a big advantage in a fight, there are generally two logical weaknesses that can let people overcome this ability.
The first is by simply being so much stronger and faster than your opponent that they can't react or defend against your attacks. After all, it doesn't matter if your opponent knows what you're going to do next if they physically can't stop you or protect themselves from you. The second is setting up a scenario that's basically the same as the first by simply outlasting your opponent and waiting for them to be too injured or tired to keep up with you and unable to defend against you.
For the second strategy, having Super-Toughness or being Nigh-Invulnerable would be a must because it doesn't matter if they can keep hitting you if they don't have the power to hurt you. In some instances you could also hit them with something that can't be avoided because they won't have time to get outside of the radius before it arrives. This typically requires knowing that they have the power and being able to plan around it. For example, if they can see five minutes into the future, using an invisible, odorless Deadly Gas that kills its victims about ten minutes after exposure.
Why this doesn't muddle up their ability to know what you're doing now can be chalked up to the Rule of Cool. Alternately, it can be Hand Waved by saying that they're not looking that far into the future, or by noting their brain can keep up with both.
Often accompanied by Spider-Sense, which warns the user of vague danger instead of predicting specific attacks.
Compare Kung-Fu Clairvoyance where the hero seems to have this, but it's not canonically one of their abilities. Contrast with Super-Reflexes, which is simply being able to quickly react to attacks as they happen, not before.
Not to be confused for Calling Your Attacks, which is about calling out the names of your own attacks as you do them.

In Worm, the tinker superhero Armsmaster has a heads-up display in his helmet that includes a combat style simulator which — when provided with sufficient video documentation of an opponents fighting style — grants him this.
Contessa, enforcer for Cauldron plays this a little straighter, by seeing "the path to victory" and following it to completion. Her power tells her a series of moves she can make to achieve whatever result she's aiming for, and she is able to see all of the steps ahead of time. She can use this in combat by seeing the path of how to defeat her opponent, showing her the exact set of movements she needs to make to dodge every attack and hit every weak point at the most effective times. It's effective enough she describes her power simply as "I win".
The Number Man's power is a variation: instead of knowing what's going to happen, he can use his innate grasp of mathematics to calculate the region where the next attack will likely come from. This means all he has to do is be not there. Every time. It's so effective that he can fight through a swarm of hostile bugs without getting bitten. However, it is ineffective against Jack Slash, because Jack can attack in so many different possible ways that it is impossible to dodge them all.
Jack Slash is eventually revealed to possess this, as his shard can detect other shards' incoming powers, subconsciously warning him about them ahead of time or communicating with those shards to subtly sabotage the powers they're sending at him. It's so subtle that even Jack isn't aware that he has it (he thinks he just has quick reflexes and/or good luck), and the heroes remain in the dark about it until Golem sees him dodge an attack a little too quickly, one that there was no possible way he could have seen it coming.

Fate Azure Destiny: Ritsuka Fujimaru battles Admiral William, who uses a Wisdom Cube to see the future and predict Ritsuka's attacks. Ritsuka defeats him by noticing William is paying too much attention to the future and not to his surroundings and tricks him into stepping into a narrow space that restricts his movement. William has no choice but to leap into the air to dodge an attack, meaning he cannot dodge the next one that defeats him.

In Bleach, Yhwach, the Big Bad of the series’ second half has this as his signature power. "A of the Almighty" lets him see into the future, however, what is unique about his foresight is that the future that he sees is not something set in stone, so other users of the Almighty are prone to confusion if a foreseen future spontaneously changes. However, not only is Yhwach able to see every possible future and follow any alterations, he is also able to bring any outcome he has foreseen immediately into the present reality. He uses this power to literally defeat his opponents instantly. His ability does have a few weaknesses. One is that he can't see or affect the past so, though he destroyed Ichigo's sword, Tsukishima used his ability to alter the past to change things so the sword was never broken. Next, while his power to show the possible futures is infallible, his own perceptions of reality are not and his senses can be fooled by Aizen's illusion abilities to see false futures created by Aizen's ability. He also cannot see futures that involve beings that use the same type of power he has with strength on his level like when the right hand of the Soul King attacked him, he was surprised because he saw no future involving the attack.

In Doom Patrol, Negative Man (nee Flash Forward) can see the immediate future but due to his relative wimpiness his only use in a fight is coordinating his teammates based on his visions. Batgirl (2000):
Cassandra Cain has a version of this due to being raised as a Tyke Bomb. Specifically, she can read human body language as easily as others read words, and thus knows what opponents are going to do almost before they do. Coupled with her incredibly fast reflexes, Cassandra is nearly untouchable in normal combat. However, the downside is that her fighting style is heavily reliant on it, so she tends to struggle against nonhuman or cybernetically enhanced foes, and at one point a psychic accidentally disables it, rendering her nearly useless until it gets restored.
Cassandra's mother, Lady Shiva, also has it, though for her it's developed from her decades of experience and complete mastery of martial arts.

In Naruto, upgrading to the three-tomoe state of the Sharingan gives this ability. If they're looking at someone in motion it will be like they can see exactly where and in what position that person's entire body will be in. However, there are some things, like the Animal Battle Aura of a tailed beast's host or Wood/Sand techniques, whose movement it can't predict very well. The other shortcoming is that knowing exactly what your opponent is about to do doesn't necessarily mean you're strong enough or fast enough to stop him from doing it. It's fairly useless against Ninjutsu specialists given that hand seals do different things depending on the sequence or element being used allowing the Sharingan user to be caught off guard. Averted the first time this ability is shown: instead of predicting and copying Zabuza's Water-based attack to execute it faster than Zabuza himself as Zabuza initially thinks to be the case, Kakashi puts him under an illusion so Zabuza is the one mimicking Kakashi's movements.
Its other weakness is if an opponent's attacks are so utterly random as to be completely unpredictable. As Killer Bee's seven-sword fighting style demonstrates.

In Fullmetal Alchemist (manga and Brotherhood), the homunculus Wrath has a super power called the Ultimate Eye, which enables him to predict every single outcome of every single situation in his field of vision. It makes him unbeatable in combat. He gets defeated when one of the heroes stabs him through the body of a fatally wounded ally, which provided cover and prevented the power from warning Wrath of the danger. That isn't enough to kill him, but another of the heroes uses the opportunity to put out his Ultimate Eye, and as a result of that and his wounds, Wrath is defeated and killed in his next battle.

Kamen Rider Zero-One gets a lesser form of this early into the show, letting him create multiple holographic duplicates of himself with different angles of attack and pick the best one to be where he actually is. His final form steps it up into the full thing, to the point where he can out-predict an opponent who also has clairvoyance, since both have identically advanced supercomputers in their heads, but Zero-One has an extra human mind on top of that, giving him just a little more processing power.

Fist of the North Star:
Hokuto Shinken practitioners have this ability thanks to Awesomeness by Analysis: after seeing your first few moves (or, with the really powerful opponents, a few minutes of fight), they can predict perfectly what you're gonna do, and react to that. This is best shown during the fight between Rei and Raoh: after seeing he's completely outmatched, Rei decides to launch an all-out attack that will kill Raoh but leave him completely exposed to Raoh's equally lethal attacks. Rei launches his attack, and Raoh defends himself, and both die. However, it's immediately revealed that this seemingly simultaneous death was Raoh playing it all in his mind, having figured out what Rei was going to do the moment he got himself in position to jump towards him! With this new knowledge, Raoh manages to defeat Rei by throwing his cape into Rei's path, preventing him from attacking, then sealing his eventual death with a single finger. This is a rare moment in Anime where asking the others to lend him their power didn't work for a hero.
That said, it was subverted a few times: aside for the fights between Hokuto Shinken practitioners (where the prediction ability of both fighters cancel each other out), we have Souther (who not only was practically immune to Hokuto Shinken, but had a few tricks Kenshiro failed to predict in their rematch) and Juza (who practices Confusion Fu to the point he's impossible to predict. While he fights remarkably well, he still loses because Raoh is that much stronger than him).

unOrdinary: Juni's "Flash Forward" ability lets her see what the outcome of an action would be for a brief bit forward in time, letting her know what her best option would be in a fight. Against a strong enough opponent it's not much use, since if all her options she can flash foward to end in her getting beaten down she's got nothing to fall back on, but it does help her in most fights.

The short-lived villain Mr. X (two-time winner of the Least Original Name prize) relies in combat on the fact that he can read people's minds and tell what they're about to do. Oh, and the fact that he's a kung fu master, but it's his ability to predict moves that gives him the edge he needs. The problem with Mr. X's ability is that it only lets him read his opponent's mind, but not to understand it. Wolverine manages to beat him by flying into a rage that he can't process, Amadeus Cho thinks in formulas and Sesquipedalian Loquaciousness to confuse him, Iron Fist uses unpredictable Drunken Boxing, Songbird hits him with an undodgeable area attack, and Quicksilver just moves and thinks too fast for him to keep up (while mocking Mr. X about how he'll know every attack is coming but be unable to do a damn thing about it). Scourge and Deadpool also give him a hard time by virtue of being insane. Basically, after his first couple of arcs, his role in the Marvel Universe is to be a whipping boy so that more popular characters can show off their guile.

Shulk gets the ability to see glimpses of the future after taking hold of the Monado, allowing him to Screw Destiny on a regular basis. Due to Gameplay and Story Integration, the party can get visions of would-be lethal attacks in battle, allowing the chance to find a way to make the attack less deadly (protect them via Status Buff, have another take the hit, etc), delay the attack, or even shattering the future outright by quickly dispatching the enemy. In his appearance in Super Smash Bros., this translates as a Counter-Attack ability.
The Telethia possess the ability to read minds, and use this to not only make sure their attacks are always accurate, but to perfectly dodge anything headed their way. This trumps even Shulk's future sight: the first time he encounters them, he foresees their attacks and moves to dodge, but at the last second they alter their aim and hit him anyway. Fortunately, Shulk quickly learns that the Monado has another power that can turn off the Telethia's mind reading powers.
Xenoblade Chronicles 2: Mythra has a power called Foresight, which lets her and her driver see a few seconds into the future. It renders them basically invincible in a fight; the only story reasons she ever loses is when it is literally impossible for her to win (such as when her opponent is moving at the speed of light). In gameplay, this manifests as a hefty bonus to both accuracy and dodge for the whole team when she's at full affinity. Since the game takes place in the same multiverse as the first Xenoblade, it's implied that this is the same power that Shulk uses with the Monado, and that the Monado is the third Aegis, Ontos, lost in a dimensional rift long ago. In her appearance in Super Smash Bros. Ultimate, this manifests as an attribute of her dodging abilities: performing a dodge just as an opponent's attack would hit grants her a moment's invulnerability while slowing down her opponent during that same instant.

Fate Series: The Fate franchise has this codified in several different skills a Servant can possess:
"Mind's Eye (False)" and "Mind's Eye (True)". Those who possess either skill have the ability to predict an opponent's moves and counter them with such speed that they appear clairvoyant. The difference between the two is that the "False" variant is an inherent trait, something the Servant was born with, while the "True" variant is something the Servant learned over a lifetime of battle experience.
"Instinct/Intuition" is referred to as the innate ability to always identify "the best possible course of action" in a fight, and at sufficiently high ranks it can even be used to "predict" what actions will best serve the owner in combat. It can even be used to predict the passage of projectiles through the air and counter surprise attacks.
Gilgamesh has Sha Naqbu Imuru, which gives him all-seeing clairvoyance. Certain factors like the cursed Grail Mud can block it. Other than that, it should theoretically make him unbeatable, but he usually only uses it for mundane purposes like cheating at chess and card games. In battle, he only uses it the first time he meets an opponent to assess what they can do and then turns it off so his fights don't become boring. Even so, his incredible arrogance means that he ignores seeing possibilities where he loses, believing it is impossible for him to lose, and since he doesn't use Sha Naqbu Imuru when he meets opponents again, he can get caught off guard if they managed to improve themselves in some way. This leads to his death against Saber in the Fate route, Shirou Emiya and Archer in the Unlimited Blade Works route, and Sakura Matou and the Shadow in the Heaven's Feel route.
Xiang Yu's "Future Prediction" is a ramped up version of even the previously mentioned skills because it's explicitly stated to reach beyond simply reading the battle or an opponent's moves and reacting at superhuman speeds to compensate right into the realm of precognition.

A few spells do this by giving their caster damage resistance.
Not just spells, but various feats, class/race abilities, skills and item properties work as boosts to Initiative rolls, keep you from being subject to surprise attacks, be able to use interrupting abilities, move out of the way, have someone else take the damage, or otherwise ticking off the GM.
This is also what numerical "insight" bonuses represent — knowing where an opponent's attack will land makes a character harder to hit, and knowing where an opponent's weak spots are going to be positioned makes a character better at hitting him.
The foresight spell gives its recipient a limited ability to see the future. This makes them impossible to surprise, and provides other benefits that vary between editions.
From 3rd Edition's Planar Handbook: On the Ever-Changing Chaos of Limbo, the Monastery of Zerth'Ad'lun teaches githzerai monks a Supernatural Martial Art called "zerthi" that allows seeing the immediate future to anticipate an opponent's moves in combat. Non-githzerai can learn zerthi through the Planar Touchstone feat, although they must first prove their prowess in a barehanded duel against one of the monks, and then abide by their grueling schedule training for a week.

Cradle Series: Every Abidan has a Presence, an Artificial Intelligence that collects and collates information. One of its uses is for combat simulations. Of course, since every individual Abidan is a Physical God, this function is rarely needed — and if it is needed, your opponent is quite likely so far beyond you that your Presence isn't much help. Lindon finishes the Monarch Northstrider's AI project, which was an attempt to replicate a celestial Presence, and ends up with one of his own. While it's much weaker since it hasn't had a chance to consume planets worth of information yet, it's still extremely useful since he's more likely to face people around his own power level.

Averted in the Dune series, where several people can predict the future. Mentats can predict the future by way of "projecting" the possible outcomes of a given choice and choosing the most logical one as the one most likely to happen, but their role was not usually that of a front-line combatant. Additionally, they are not actually prescient and are not always right. The Kwisatz Haderach and the Space Navigators do have actual prescience, but yet when it comes to combat, prescience is useless because of too much happening too fast. During one fight, the most Paul could see was that possible futures include him winning and him being carried out as a corpse. This is because the power does not necessarily predict possible outcomes individually on an action by action (or reaction by reaction) basis but rather works by giving visions that are years-long based on a chain of events that could be triggered by a course of action. Additionally, prescience can be nullified by the presence of other prescient individuals, and some individuals and objects are immune to prescience.

God of War Ragnarök has Heimdall, the God of Foresight, and he uses his godly power to avoid any upcoming attack on him with a nonchalant dodge or block. The first fight against him is completely hopeless as a result, and a decent chunk of the game is spent finding a way to actually hit him. Kratos does obtain the Draupnir Spear for this purpose, and realizes that while Heimdall can see into the future to assess how to move, he's still limited by his ability to physically react, and is generally too smug to bother keeping a safe distance. It quickly becomes a matter of Kratos reading Heimdall's patterns and overwhelming him to the point he can't keep up, and once Kratos manages to land a single blow on Heimdall, he quickly loses his cool, becoming much more aggressive but also much sloppier with his use of foresight.

The Answer Talker is an ability caused by 'dying and returning to life,' which he gained due to years of experimentation by a Mad Scientist, and allows the user to immediately glean the answer to any question they ask, as long as they can see, hear, or otherwise perceive the subject of their question...and keep their focus. The latter is a weakness that Dufort exploits against Kiyo when he gains the Answer Talker, too.

The ZERO System from Mobile Suit Gundam Wing works like this, except that it's technological rather than a superpower. It's an advanced combat analysis complex that puts together massive amounts of battle data and tactical analysis and feeds it directly into the pilot's brain, including predictions of what the enemy's going to do. Combined with the mindlink the System creates, this allows the pilot to move and react as if the Humongous Mecha were part of his own body. The problem is that it tends to evaluate everything as a potential threat, and to assume all potential threats are imminent threats, meaning that often the ZERO System's recommended course of action is "destroy everything in sight". Combined with the sensory overload of all that data being dumped into the pilot's brain, it takes considerable willpower and emotional control to use the ZERO System without going insane. Heero, Zechs and Quatre all master it to the point that they can use it with no problem at all by the end of the series, and Dorothy is able to instantly use it without hindrance, perhaps on account of already being a bit crazy. In the final battle, Quatre somehow seems to still be able to do this even after he disconnects the ZERO System from his Gundam. Despite Psychic Powers supposedly not existing in Gundam Wing.

Shulk gets the ability to see glimpses of the future after taking hold of the Monado, allowing him to Screw Destiny on a regular basis. Due to Gameplay and Story Integration, the party can get visions of would-be lethal attacks in battle, allowing the chance to find a way to make the attack less deadly (protect them via Status Buff, have another take the hit, etc), delay the attack, or even shattering the future outright by quickly dispatching the enemy. In his appearance in Super Smash Bros., this translates as a Counter-Attack ability.
The Telethia possess the ability to read minds, and use this to not only make sure their attacks are always accurate, but to perfectly dodge anything headed their way. This trumps even Shulk's future sight: the first time he encounters them, he foresees their attacks and moves to dodge, but at the last second they alter their aim and hit him anyway. Fortunately, Shulk quickly learns that the Monado has another power that can turn off the Telethia's mind reading powers.

It renders them basically invincible in a fight; the only story reasons she ever loses is when it is literally impossible for her to win (such as when her opponent is moving at the speed of light). In gameplay, this manifests as a hefty bonus to both accuracy and dodge for the whole team when she's at full affinity. Since the game takes place in the same multiverse as the first Xenoblade, it's implied that this is the same power that Shulk uses with the Monado, and that the Monado is the third Aegis, Ontos, lost in a dimensional rift long ago. In her appearance in Super Smash Bros. Ultimate, this manifests as an attribute of her dodging abilities: performing a dodge just as an opponent's attack would hit grants her a moment's invulnerability while slowing down her opponent during that same instant.

It works quite well when the Calirath is going to be risking death in a battle — the stronger Caliraths can even use it to fight with, and the visions grow stronger if the person has a good chance of dying. There are multiple ways to achieve this in Kenichi: The Mightiest Disciple. With the Inner Eye, a martial artist memorizes the rhythm of the opponent's movements, and can predict where the opponent will move and attack, making their attacks nearly always accurate. The Ryusei Seikuken technique reduces the size of a Seikuken to a thin layer over the skin, letting the user dodge with just the smallest movements, however to the enemy it looks like their mind is being read. And the Attack Trail Fight, where you form a trail of all possible attack patterns the opponent may take, having, attempting to read the next trails and block them. Needless to say, all three are very high level techniques.

One Piece:
This is the foremost ability of Observation Haki, also called "Color of Observation" or Kenbunshoku Haki, one of three will-based superpowers. It allows the user to sense the presence/aura of other living things, and reading that aura lets them predict their opponent's next move. The ability is rare and powerful, but it has three weaknesses: the user has to maintain focus, the opponent has to know where the attack is going to land, and most importantly, predicting the opponent's attacks is useless if you can't do anything to stop them.
It first appears as Mantra in Skypiea; Enel, his four Priests, and Aisa the Shandoran all have some form of the ability. Enel is even able to enhance this with his lightning powers, combining Mantra with electromagnetic waves to eavesdrop on whomever he pleases.
Among the Straw Hats, Luffy, Zoro, and Sanji developed it during the Time Skip—with Sanji specializing in it—and Usopp awakened it in the Dressrosa arc.
Charlotte Katakuri, one of the Sweet-Three-Generals of the Big Mom Pirates, has supposedly trained his Observation Haki to the point that he can actually glimpse slightly into the future, as long as he maintains his composure. This ability, among others, makes him one of the most dangerous pirates shown thus far, with a bounty racking up over a billion. In fact, it inspires Luffy to keep fighting Katakuri for the purpose of strengthening his own Observation Haki to gain this ability as well. It works.

It's used to justify the existence of checkpoints in PointDevice mode: your characters took a medicine that lets them see into the future, and as such aren't dying and retrying, but simply finding a possible future that doesn't result in them dying. However, you can choose to not take the medicine by selecting the Legacy Mode (the signature classic mode).
In one of Sanae's endings, it is noted that taking the aforementioned medicine would have been deadly to a native god like Suwako, and if Sanae used it, nothing deadly would've happen, but her impurities would be washed away, leaving her a "pure human" as a result.

The short-lived villain Mr. X (two-time winner of the Least Original Name prize) relies in combat on the fact that he can read people's minds and tell what they're about to do. Oh, and the fact that he's a kung fu master, but it's his ability to predict moves that gives him the edge he needs. The problem with Mr. X's ability is that it only lets him read his opponent's mind, but not to understand it. Wolverine manages to beat him by flying into a rage that he can't process, Amadeus Cho thinks in formulas and Sesquipedalian Loquaciousness to confuse him, Iron Fist uses unpredictable Drunken Boxing, Songbird hits him with an undodgeable area attack, and Quicksilver just moves and thinks too fast for him to keep up (while mocking Mr. X about how he'll know every attack is coming but be unable to do a damn thing about it). Scourge and Deadpool also give him a hard time by virtue of being insane. Basically, after his first couple of arcs, his role in the Marvel Universe is to be a whipping boy so that more popular characters can show off their guile.

Hinata Sakaguchi has the Unique Skill [Mathematician], which allows her to speed up her senses and thought processes by over 1000x, analyze the battlefield and the opponent, and even predict the paths of her enemy's attacks.
During his second battle with Hinata, Rimuru Tempest is forced to evolve a similar Skill in order to counter the knight's predictions and obtains [Future Attack Prediction], allowing him to perceive the possible moves an enemy can use and determine which is the most likely form of attack.
Laplace has the Unique Skill [Future Vision], which allows him to look ahead several seconds into the future at will. He uses this to supplement his Confusion Fu fighting style by checking what possible moves will have the best effect on his enemy, or see dangerous attacks incoming and already prepare himself to dodge them. It also works wonders for letting him know when he's outgunned and thus when it's time to run.

Cobra uses his advanced magical hearing to achieve this via hearing his opponents' thoughts and react accordingly. Even if they somehow shut him out, he can still use his hearing to less accurately predict their movements based on body functions like heartbeat, breathing, and muscle movements. The only enemy who could completely No-Sell this was Acnologia, whose Anti-Magic rendered his mind as "total silence" to Cobra's hearing on top of being physically superior in nearly every way such that Cobra couldn't counter by listening to his other body functions.
In the climax of the Grand Magic Games, Ultear had hoped to use the ultimate form of her time powers to turn back time for at least a few hours so the heroes can have a better chance at beating the invading dragons who at the moment were on the verge of killing many of them. It only turns back time for a few minutes, but in the process, temporarily granted the heroes this power to avoid and counter deadly attacks and ambushes.

This is part of the reason that Eldar psykers are effective in close combat. The other reason is the rest of their repertoire of powers usually involve eldritch lightning or psychic flames.
On a more strategic level, Eldar commanders use this to guide their warhosts to victory. Often, warhost commanders tend to be senior seers or aided by senior seers, who skein the threads of fate to determine what the best course of action is, and then they telepathically send this information through to lesser seers and warlocks, who function as squad leaders. Because of this, Eldar warhosts move and react to enemy action with an almost uncanny and very confusing degree of alacrity.
Averted for Kairos Fateweaver, Oracle of Tzeench — he is possibly the greatest seer of the universe but his powers are only useful in long-term Xanatos Roulettes; he is actually blind to the present (one head sees the future and the other the past, and the one that sees the future doesn't see it close enough to notice oncoming attacks) and so physically a pushover by greater demon standards.

Subverted in Assassins of Brotherhood, in which the hero, Wei, faces his former brother-in-arms after surviving a betrayal. The ex-brother had predicted Wei's moves, and played out in his mind how he will defeat Wei by forcing Wei to move to his back before taking down Wei with an Offhand Backhand (he even have an Imagine Spot where he utterly obliterates Wei in a twelve-second Curb-Stomp Battle). But Wei, having survived their initial assasination, and having spent several months with the monks who rescued him, achieving inner peace in the process, no longer practices the same fighting moves the assassins are trained with — in the ensuing duel the Curb-Stomp Battle has Wei emerging the winner, killing his brother via Slashed Throat.

Teen Titans:
In one comic, Clock King is able to clobber Robin by virtue of being able to see a few seconds into the future and knowing what his next move in combat is going to be.
Ravager has a similar power, though it's a bit unclear as to how. In the final issue of Terror Titans, she wipes the floor with Clock King by fighting dirty and not giving him time to react to his visions. At first, it's stated to be psychic in nature, then later retconned into being a side-effect of the Super Serum she took which allows her to anticipate moves based on body language, similar to Cass, above. Her father Deathstroke, who shares the same powers as Ravager, explains this to her; it's likely that Deathstroke has the ability as well, due to taking the same serum decades ago, but it's not really talked about as much with him.

Midnighter has enhanced strength, speed, durability, and healing, but he's not invulnerable. He once tried this schtick on Captain Atom, a Physical God. Atom was not impressed.
Additionally, he once faced off against a guy named Forethought, who had actual Combat Clairvoyance. He wiped the floor with Midnighter; no matter what Midnighter tried, Forethought would simply see the attack coming and deflect it. Midnighter beat him in their second match by doubling his body armor on specific spots, then letting Forethought wail on those points. When Forethought (who had no super strength or endurance) finally wore himself out, then Mindnighter kicked the crap out of him.

In Mistborn, the metal atium grants Combat Clairvoyance to those who can use it (mostly the titular Mistborn). In-universe, someone burning atium is believed to be completely unstoppable, except by someone else also burning atium. Normal use allows the user to see an image of exactly what the opponent is about to do, as well as allowing them to instinctively process and react to the information, even being able to dodge attacks from behind. When both combatants are using atium, they are flooded by an array of constantly shifting futures as they react to each other's movements and reactions, and see several shifting images of each other as a result. Despite the power it grants, each book in the original trilogy highlights a weakness of atium. It's possible to stop burning atium to feign running out then burn it again when the enemy acts, leading to an exploitable moment of confusion. Also, while atium can predict the actions of the opponent, it does not predict the possible outcomes of the user's actions, meaning that atium-use relies on the opponent acting first. Thus, another weakness is that the power can be tricked by deciding on a course of action, and then once the atium-user foresees the action and reacts to it, to quickly switch gears and react to the atium-user's reaction; this has the effect of the atium-user seeing the opponent's before-image "split". The last weakness is that no matter how much atium an Allomancer has they can still be overwhelmed by exhaustion if they fight long enough against too many enemies. There's also another metal, electrum, which allows the user to see the shadow of their own possible future actions; this is less useful because it always gives the shadow-splitting effect of two atium-users facing each other, and doesn't let you see incoming attacks, just yourself getting hit by them. Despite this, electrum counters atium the same way that it counters itself, and is much cheaper.
