When Ai and Makoto start hooking up in one path, she assumes that they're dating and he assumes that they're not. Of course, it leads to issues.

Then he actually starts going out with Setsuna. When he finally gets to what would normally be Setsuna's good ending, the two mothers are in the background musing at how unlikely this development was and ceding 'first place' to Setsuna before starting to argue about who gets to be second. While there are several endings that tease like this, this one actually confirms it, with Mai, Setsuna and Youko all pregnant and suggesting that Sekai join them. In one ending, Makoto gets together with both Kotonoha and Kokoro. Another variant of the same ending has their mother in on it too.

In the present, though, even setting aside Otome sabotaging her she doesn't have a chance anymore, as shown in one ending. The driving conflict in the last chapter is that Makoto believes Setsuna has been in a secret relationship with Shun. She's not; she's close to Shun because he's her father. Whether Makoto figures out the truth and how he reacts to it is one of the biggest factors for which ending is achieved.
