When the birth surname is changed to that of their adoptive family's, it can be a sign that they're a part of said family. When the given name is changed to something else, it could be because the initial name was an Embarrassing First Name or otherwise inappropriate; perhaps the new guardian simply didn't like the original name, or never even knew the original name of the mysterious Doorstop Baby.
