Countries have flags. This fact is fairly important, given a flag is one of the most recognizable symbols a country can have. So if one wanted to make a well-established Fictional Country, it's only natural that country gets its own flag as well. And with symbolizing a fictional country comes many intentional design choices, often inspired heavily by design trends in real-life flags.
Where exactly flags originally came from is difficult to say. There are many possibilities, ranging from early Egyptian nobility to Roman military standards. The modern use most likely stems from medieval banners and heraldry.
The purpose of a flag in any setting is for it to be easily identified by those who see it. As such, flags tend to be brightly colored and utilize simple designs. There is sometimes imagery on them, but complex pictures are more difficult to embroider.
Typically flags are influenced by their nation's history. Even if they are simply colors in geometric shapes, they are quite often colors that were significant at a defining moment. Others might have symbols related to their founders, heroes, myths, or other significant imagery.
Sometimes creators will blatantly invoke real-life flags to get their point across. A red, white, and blue flag will represent their version of Eagleland, especially if stars or stripes are involved. Red flags or ones that use images of workers' tools will mean it's the flag of Commie Land, and some will display Red and Black Totalitarianism. Other times works that are 20 Minutes into the Future may take real flags and modify them to reflect changes that have occurred between the present and whenever this is, such as adding stars to the US flag, a relatively easy change.
In all cases, there is the possibility of their enemies expressing their dislike by burning it.

Curtis Tarver's sports and marching band blog/YouTube channel 80 Minutes of Regulation has a flag that is a white-red-white horizontal tricolor with the logo in the middle, a drum seen from above with the number 80 on it.
Bradley Lockhart has created a wide variety of orignal flags, most commonly related to his home state of Washington. Aside from his officially adopted flag of Bellingham, he has made various proposals such as flag designs for Washington, Olympia, and Seattle. The latter he created two designs for, the most famous being the Orca Face flag. He also made a conceptual redesign of the American flag for a contest based on the lyrics of "America the Beautiful".

The People's Kingdom of Great Britain replaces the Union Jack with the "Asterisk of Liberty", a version of the Union Jack where the St George's cross and St Andrew's cross are both purple, on a white field. The reformed Kingdom of the Britons has the asterisk in the centre of the Union Jack, then the Kingdom of England drops it in the wake of the Pandoric War. This also sees an independent Scotland, which keeps the Union Jack but puts the St Andrew's cross over the St George's.
The various Jacobin Republics of the 18th century favoured turning the existing flag upside down and recolouring it black and red. The Republic of Portugal is the only country where this iconography survives.
The Societist Combine has a black flag with the Threefold Eye (an equalateral triangle with each side extending slightly, and a dot in the middle), despite the fact national symbols are exactly what Societism is supposed to be against.

The Empire itself has no single flag but uses a range of iconography including skulls and griffons, as well as symbols of specific meaning like a warhammer (after Ghal Maraz, the famous weapon of their deified founder Sigmar, which is wielded by the Emperors), a comet with two tails (a cosmic phenomenon that the Empire associates with Sigmar) and the imperial cross (resembling the German iron cross, symbolic of unity and a famous battle in the Empire's founding). The provinces and city-states of the Empire each have their own flags, with symbols relevant to their own histories.
The Bretonnian dukedoms each have traditional heraldry, used by their lords and, in modified form, by the lord's sworn knights and vassals. Examples include Parravon's gold pegasus on black, derived from its famous pegasus riders; l'Anguille's blue sea monster on white, representing the beasts that live along its coasts; and Bastonne's red dragon on gold, in honor of its founder, Gilles le Bretton, who among other things was a famous dragonslayer.
Each High Elven kingdom uses iconography derived from some notable trait or tradition, such as Chrace's white lion head to represent its wild lions, Caledor's coiling dragon in honor of its Dragon Riders, or Ellyrion's white horse head to represent its equestrian traditions.
Ogres as a whole use a stylized ring of teeth to represent the Great Maw, their god. Each tribe uses a modification of this as its specific symbol — the wealthy Goldtooth tribe has a Maw with a golden fang, the Feastmasters have a cauldron ringed by teeth, and so on.

ArmA: Armed Assault: The flag of the Kingdom of Sahrani ("South Sahrani") features a vertical Triband with the country's coat of arms in the middle, a grifon circled by a laurel wreath; "North Sahrani" (the Democratic Republic of Sahrani) has a flag divided between a diagonal line from the bottom right side, with black on the left and orange on the right, with the country's crest in the middle, a socialist star circled by wheat and cogs.
Arma 2 (and its Spin-Off DayZ) set in the nation of Chernarus features a complex flag, it's all green, with a big yellow star circled by smaller stars in the canton, and in the bottom half features divergent white stripes; the country's military (The Chernarus Defense Forces) have simple horizontal bands, yellow at the top and green at the bottom. The ChDKZ rebels have a horizontal tricolor of green, black, and red with a red star in the middle; and the independent NAPA guerrillas have a flag with a black smaller hoist and green fly, with crossed swords where the colors meet.
Arma 2: Operation Arrowhead shifts the setting to Takistan. The country (and their army) have a horizontal tricolour of white, black, and green, with a stylized star-and-crescent; the Takistani Rebels have a flag with a black smaller hoist and green fly divided by a serrated line, while Takistani Militia has a simple black flag with a star-and-crescent.
Arma 3: The Republic of Altis and Stratis is a green flag with a yellow isosceles triangle superimposed on the larger white-edged black triangle, both based on the hoist-side, pointed toward the fly-side. The Altis Armed Forces (AAF) have a flag with the Altis flag in the canton, and the coat-of-arms of the military (a lion with two swords below it) in the center; the rebel FIA faction use a flag similar to Altis', but with cyan blue and a lion in the host side. The CSAT faction uses a red flag with six black hexagons in the form of a larger hexagon.
Arma 3: Apex: The flag of Tanoa is a blue field with the Southern Cross in the canton and green, white, and yellow rays coming from the bottom left.
Arma 3: Contact: The nation of Livonia has a blue, white, and yellow triband with the country's coat-of-arms in the middle, a white stag inside a blue shield. The Livonian Defense Forces (LDF) have a blue field with said coat on arms in the middle, surrounded by laurel wreaths, two rifles, and the motto "Siła i Czujność" ("Strength and Vigilance") on the bottom.
